When the patient has an extreme toothache from chewing or when pressure is applied, tooth discoloration, sensitivity to heat or cold, swelling or tenderness of the gums, or a pimple that is persistent or recurring on the gums, it may mean the tooth is infected and the dentist may recommend a root canal therapy.

Root canal therapy removes the nerve and the infected pulp of the tooth, and then seals it to protect it from further harm. If the tooth is left untreated, bacteria may cause the bone and surrounding tissues to become infected. An abscess, in which the infection has spread past the roots, may form. An abscess may lead to swelling that spreads to the face, neck or other areas of the head, and even bone loss around the tip of the root.

Root canal therapy has had a bad reputation over the years from the public, with many bad jokes referring to how painful the process is. The truth is that getting a root canal is no more painful than getting a tooth filled. A patient may need one or more office visits, depending on the severity of the case. A dentist or endodontist may perform the procedure.

An X-ray will be taken to see the shape of the root canals and determine if the surrounding bone has been infected. A local anesthetic will be applied and an access hole will be drilled into the tooth. The pulp, the nerve tissue, bacteria, and any related debris will be removed using root canal files. Water or sodium hypochlorite is used to flush the area. If the tooth was infected, the dentist may choose to place medication in the tooth, and use a temporary filling. When it is time for the tooth to be sealed, the sealer compound will be placed in the root canal, and a filling is placed on the exterior tooth. If there is any further restoration work needed for the tooth, the dentist will discuss options with the patient.

The first few days after a root canal procedure, the patient may feel sensitivity in the tooth. Over the counter medication such as Tylenol or Ibuprofen can usually control any pain or discomfort. Most patients can return to their normal routine the next day. It is recommended, especially if there is a temporary filling, to minimize chewing on the tooth undergoing the therapy. After the procedure, the patient should be able to continue normal oral care and regular dental visits.
